Right ventricular-coronary artery fistula. This is a magnified image of the right ventricular outflow tract from the subcostal acoustic window. The arrow is delineating a color jet of to-and-from flow within the myocardium and right ventricular cavity. This is the typical appearance of a low velocity fistula due to intramyocardial sinusoids which communicate with both the coronary arteries and the ventricular cavity.
