Note the proximal convergence of the regurgitant flow which forms isovelocity lines (PISA) as it approaches the regurgitant orifice from the aorta. The vena contracta can be seen as the narrowest region of the regurgitant jet at the regurgitant orifice. The relatively large width of the vena contracta in this patient is consistent with severe regurgitation. The regurgitant jet is large and extends far into the left ventricle. Note that the jet is eccentric and the regurgitation is more severe than depicted by jet area alone.
