This is an apical 4-chamber view from a fetal echocardiogram demonstrating a restrictive atrial septum by 2D imaging. Note that the septum bulges from the dilated left atrium toward the right atrial. In addition, the pulmonary veins are very dilated. This suggests elevated pulmonary venous pressure and is an ominous finding. If this is found early, a case like this may be a candidate for fetal intervention. If found later in gestation, an immediate postnatal procedure to compress the left atrium should be considered.
